Case Management
At the heart of our Program is our case management. Family Development Specialists provide intensive case management for clients, especially in the first few months of program participation.
SNAP will work with you to:
- identify specific goals clients want to achieve during your time in the Program,
- as well as steps clients need to take to accomplish the goals.
Clients will also be asked to:
- work on a budget
- attend life skills classes
- attend monthly case review meetings
Clients will be housed based on follow through with program rules and requirements.
Within the SNAP Homeless Program are several different housing subprograms. Some of the subprograms provide short-term, emergency housing while others provide longer-term transitional housing. Each one is slightly different and in total they create a stepping-stone to help clients achieve self-sufficiency.
